- Have United HealthCare coverage through ADNY
- Be at least 18 years of age
- Your office visit must be coded as an Annual Physical

Log into your MyUHC.com account
Click “My Health & wellness”
Click “Get Started Now”
Click “Sign Up for Rally”
Enter your email and click “Continue”
Follow the prompts to create your Rally account:
Now that your Rally account is set up you will be prompted to take a survey.
