2025 Form 1099-R Information
The 2025 Form 1099-Rs were mailed by January 31, 2026 along with the January 2026 Direct Deposit Summary (for those who receive directly deposited funds) or your January 2026 Payment Summary (for those who receive a physical check). If you do not receive your 2025 Form 1099-R by Tuesday, February 10th, please contact 646-794-3030, prompt #1. Please do not contact the Pension Office prior to this date.
Change of Address Request
