Registration for NY Catholic Youth Day 2024 is CLOSED. We are unable to accept additional registrations at this time; we are also unable to accept walk-ins on-site. Thank you for understanding.

- April 27th Youth Participant Consent Form English | Spanish
- April 27th Adult Participant Consent Form English | Spanish
- April 28th Youth Participant Consent Form English | Spanish
- April 28th Adult Participant Consent Form English | Spanish
- Both Days Code of Conduct (for parish use and retention) English | Spanish
- For Adults (18+) attending as Chaperones, Volunteers or Assisting on Campus
- If you have previously completed the below with your parish, we will verify your clearance with Safe Environment – you do not need to complete it again. This is listed for ease of access for those who are new.
- Background Check Authorization Form English I Spanish

- Transportation and Drivers Policy (See pg. 21-22)
- If needed have drivers complete the “Driver Information Sheet” (Return Completed to Parish)
- If needed “Approved Bus List”
